Respecting your privacy has been mandatory since the progressive implementation of Bill 25 in September 2022 regarding personal information protection.

This policy aims to inform you about my commitment to respecting privacy and protecting personal information. It outlines my practices related to the collection, use, disclosure, retention, destruction, and protection of your personal information. It applies to the personal information I hold about you, which directly or indirectly identifies you.

1. Collection and Use of Personal Information

1.1 Methods by which Personal Information is Collected:

The personal information collected is provided directly by you, either verbally during your appointment or through the completion of fields on the appointment booking page. Specifically, when:

  • You respond verbally to questions when opening your file and for subsequent follow-ups.
  • You communicate with me via email or text messaging.
  • You browse the clinic’s website.

1.2 Use of Personal Information:

I commit to using only the personal information necessary and for the purposes for which it was collected. Here are the categories of personal information I may collect from you and the purposes for which it is requested:

1.3 Access to Your Personal Information:

The personal information collected is only accessible by me, Martine Oppliger, exclusively.

2. Disclosure of Personal Information

Your personal information will never be shared with any third party, except in the case of verification by your insurance company. In such cases, I will only confirm the dates of your visits without providing details about the reason for your consultation or the treatment administered. This will only be done after you have been properly identified by the insurance company and not the other way around (I do not confirm who came on a specific date, but the insurance company requests me to confirm if a properly identified person attended on specified dates).

3. Retention of Personal Information

Personal information is stored on cloud-based tools that meet the highest Canadian privacy protection standards.

4. Destruction of Personal Information

As required by the ANAQ, I store your personal information securely for a minimum of 7 years after the last registration or insertion to comply with article A.VI.02 of the ANAQ Code of Ethics. Once the retention period for personal information has ended, I ensure it is securely destroyed using a method that guarantees permanent deletion, making reconstruction impossible.

5. Measures for Protection of Personal Information

To ensure the protection of your personal information, a password is required to access all my devices where I store or review your data.
